Candidates
People can consider a blepharoplasty if they have :
- Loose or sagging skin that creates folds or disturbs the natural contour of the upper eyelid, sometimes impairing vision
- Fatty deposits that appear as puffiness in the eyelids
- Bags under the eyes
- Drooping lower eyelids that reveal white below the iris
- Excess skin and fine wrinkles of the lower eyelid
Pre-operative
To schedule a surgery, you will need to consult an eye specialist and Oculoplastic surgeon for a routine examination. Here, you will be asked about your medical history and your main reasons for opting for the surgery. You will then likely undergo a physical examination to assess the situation, a vision examination followed by photographs of your eyelids to help in planning the surgery. Keep in mind that certain medicines should not be consumed during surgery such as anti-platlet agents etc. In case you are on any medication, be sure to tell the doctor in advance so that they can advise you if any need to be stopped before the surgery.
Procedure
The procedures are done under local anaesthesia. For upper eyelid correction, the surgeon makes an incision along the eyelid crease, removes excess fat and skin, and then closes the incision with stitches.
In the case of lower eyelid surgery, the incision may be made just below the lash line or along the inner eyelid (commonly using the transconjunctival approach), allowing for the removal of excess fat and skin before sealing the incision. If both eyelids are being addressed, the surgeon will generally start with the upper eyelid before proceeding to the lower eyelid.
Post-op care
During your eyelid surgery recovery, lubricating ointment and cold compresses may be applied, and in some cases your eyes may be loosely covered with gauze, after your procedure is completed.
You will be given specific instructions that may include how to care for your eyes, medications to apply or take orally to aid healing and reduce the potential for infection, specific concerns to look for at the surgical site or in your overall health and when to follow-up with your plastic surgeon.
Recovery
Initial healing may include some swelling, bruising, irritation, dry eyes and discomfort that can be controlled with medication, cold compresses and ointment. Irritation at the incision sites is also possible. You must practice diligent sun protection and use darkly tinted sunglasses until the healing process is fully complete.
Complications
- Bleeding from the incision lines
- Changes in skin sensation or numbness of the eyelashes
- Difficulty closing your eyes
- Dryness to the eyes
- Ectropion, an outward rolling of the lower eyelid
- Lid lag, a pulling down of the lower eyelid, may occur and is often temporary
- Possible need for revision surgery
- Sensitivity to sun or other bright light
- Swelling and bruising
- Unfavorable scarring

Most people who receive eyelid surgery report overall satisfaction and an improved appearance. Most patients never have another surgery. However, natural aging processes still occur after the surgery. Patients usually report some natural aging effects around the 5-7 year mark post-surgery.
At this time, some choose to have a touch-up surgery. This is not necessary, but it can extend the longevity of eyelid surgery results.
A double eyelid blepharoplasty or a double blepharoplasty is a surgical procedure that removes extra fatty tissue and skin from the lower and the upper eyelid.
It is a combination of the two procedures.
The eyelid heals better than almost any other part of the body. For this reason, your visible scarring will be relatively minimal. If someone knows you received eyelid surgery, they might know to look for scars, but blepharoplasty scars are typically not visible to the casual observer.
Eyelid scars are typically only visible when your eyes are closed. Even then, someone would need to heavily focus on your eyelids to see the scarring.
A blepharoplasty may treat or change the appearance of some wrinkles in the eye area. If you are concerned about significant facial laxity, you may consider looking into other procedures to combine with blepharoplasty. This can include a facelift (rhytidectomy) or lateral brow lift.
